Nominee Information Required: 1. Full name, title, complete address, telephone number, and email address of the nominee. 2. Summary of the nominee’s outstanding service to benefit or advance the field of law and national security affairs. (100-200 words) 3. Detailed narrative of the nominee’s achievements or service, including the following information: how the nominee’s performance was outstanding with particular reference to special contributions and significant accomplishments which have benefitted or advanced the field of law and national security affairs; the time frame for the service described; an assessment of the impact of the performance; include such other factors as you deem important (no more than two pages). 4. Names, titles and phone numbers of three other individuals who are familiar with the nominee’s achievement or service. 5. A brief statement about the nominator: name, title, address, telephone number, relationship to the nominee, and any other factors which you believe would assist the Awards Committee in evaluating the nomination. 6. Note to Nominators: Please inform the Awards Committee if you do not wish us to divulge your name in any discussion which may take place with the nominee or with any references supplied.
